How do I make an appointment or change my appointment?
Appointments can be scheduled during our office hours of Monday through Friday, 9 am - 5 pm. To schedule or change your appointment, please call our office at (432) 332-5557. Be sure to ask us about our concierge services if you are interested in scheduling a same-day appointment.
What type of insurance do you accept?
We currently accept Medicare, Medicare Replacements, BCBS, Cigna, Aetna and most commercial policies. We can review insurance information with you when scheduling your appointment; however, each insurance plan is very different, so we recommend contacting your insurance provider to determine if your specific plan will cover our services.
what can i expect as a new patient?
Our staff will have you fill out registration and health history forms upon arrival. We ask that new patients arrive 30 minutes before the scheduled appointment time to complete the paperwork. If you would prefer to fill these forms out online, just contact our office and we can assist you.
What do i need to bring to my appointment?
Please bring a list of medications you are currently taking and your insurance card or copy of your card. If your insurance requires a co-pay/deductible, please be prepared to pay that at the time of your visit. It is also helpful to write down a list of your medical questions so that we can ensure all of your questions are answered in the exam room.
how do i get a refill on my prescription?
Dr. Dar may need to review your medical records before prescribing or refilling any medications. Please call our office to get a refill on medications that we have prescribed.
